Terms and Policies Agreement By enrolling in the TQI diet class, you agree to the following: 1. Changes in diet can have dramatic effects on the body, and you acknowledge that you should consult with a licensed health care provider before making any drastic changes in diet. You acknowledge and agree that this is especially important for anyone taking prescription medications because dietary changes may alter the appropriate dose of those medications. 2. That the instructor, Kathy Abascal, is sharing her experience using food to quiet inflammation and promote health, that she is not a licensed health care provider and is not recommending that you in whole or in part adopt her views on food. Instead, as an educated adult, you take full responsibility for any dietary changes you make and any effect those changes may have on your health. 3. You therefore agree to hold Kathy Abascal, her agents and her employees harmless for any and all consequences of any dietary changes you choose to make. 4. Seattle class location: You are aware that there are certain inherent risks involved in participating in this program, including but not limited to the risk of theft or of damage of personal property and the risk of personal injury from participation in this program. In consideration of your being granted permission to participate in these activities and to use the facilities of Whole Foods Market #174 and/or any other activities at Whole Foods Market #174, including food service, you, on behalf of his/her self, executor, administrators, heirs, next of kin, and successors, hereby covenant to hold harmless and indemnify Whole Foods Market#174 and all its officers, departments, agents and employees as well as Kathy Abascal and her agents and employees from any and all claims (except for claims based on malicious conduct by Whole Foods Market#174 employees), lessees, damages, injuries, fines, penalties and costs (including court costs and attorney's fees), charges, liabilities or exposures, however caused, resulting from or arising out of or in any way connected to registrant's participation in the program. By submitting your registration to this program, you acknowledge that you have read and understand this HOLD HARMLESS AGREEMENT and agree to its terms. 5. Cancellation: The payment of the class tuition is non-refundable if registration is not cancelled 72 hours before the class is scheduled to begin. There is a $20 administrative fee for a cancellation 3 to 14 days prior and a $5 administrative fee for a cancellation more than 14 days before the class begins. For the online classes, due to the mailing out of class materials, there is no refund within 7 days of the start of the class. TQI reserves the right to cancel any class due to attendance or location issues. A full refund of the class fee will be made in those instances. 6. Make Up: This program consists of a series of five classes and participants are expected to attend these class in a sequence. If you are unable to attend one or more classes in the series, you understand that make-up classes may not be available. There is a $5 processing fee for each make up class requested and granted, and a request for a make up class must be made while the series in which you are enrolled is in progress. No more than 2 classes can be made up in any series and the make up class, if granted, can only be taken in an online class.
